NOTE: ANY WINNER WHO MADE PAYMENT VIA CHECK IN LESS THAN TWO WEEKS OF DRAWING MAY DELAY PAYMENT PROCESSING. THE WINNINGS WILL BE HELD UNTIL THE CHECK CLEARS (APPROXIMATELY 2 WEEKS FROM THE DATE THE CHECK WAS POSTED IN THE LTC BUSINESS OFFICE FOR DEPOSIT).
Ticket information must be complete, including name, address, email address, and phone number.
A COPY OF TICKETS PAID FOR BY PHONE CAN BE EMAILED (SCANNED) TO THE PURCHASER UPON REQUEST. ACTIVE EMAIL ADDRESS WILL BE REQUIRED.
ORIGINAL TICKET CAN ALSO BE PICKED UP IN PERSON OR MAILED IF PREFERRED BY PURCHASER.
WINNER DOES NOT NEED TO BE PRESENT TO WIN.
A W-9 will be required for all winners prior to disbursement of funds. Form can be obtained from the LTC Business Office or online at https://www.irs.gov/pub/irs-pdf/fw9.pdf
IL Form 5754 will have to be completed by the $250, $500, $1000, and the grand prize winners. This form is required as a substitute to the W-9 for these winners. IRS Form 5754 will be required to be completed by the grand prize winner ($25,000). This form is required as a substitute to the W-9 for the grand prize winner. Grand prize winner must present Two forms of ID, one being a photo ID. Acceptable forms include: Social Security Card, Valid Driver’s License, Military ID Card, Voter Registration Card, Student ID, etc.). Photocopies of each form of ID should be obtained and provided to the LTC Business Office prior to disbursement of funds.
Winnings from the drawing are subject to withholding of up to 24% for Federal Income Tax purposes and 4.95% for Illinois Income Tax purposes and winner may be required to supply IRS Form 5754, IL Form 5754.
Payment of winnings will be made within 30 days of drawing provided all required forms and/or copies of required identification documents have been provided to the LTC Business Office.
Winnings must be claimed within 90 days.

Upon approval of winner, a winning photo may be posted on LTC social media or via the LTC home page or athletic site or used in other advertising material.
